SSA/Case Management

What is Service and Support Administration(SSA)?

The SSA department will provide personalized assistance based on the needs of people that we serve.  SSAs assist with community resources and connecting through natural and paid supports.  Collaboration between the SSA, the person, their family/friends and team will provide support to reach and achieve goals and maintain what is important to the person and what is important for the person.


Contact SSA Director, Amanda Rowlinson, for initial information on the eligibility process.  You must have a signed diagnosis from a physician, psychologist or psychiatrist.  Documented diagnosis of a disability before the age of 22.  The Ohio Eligibility Determination Assessment will then be scheduled and completed.

Who can receive service and support administration?

If you are a person with a developmental disability, you are eligible to have an SSA and receive service and support administration when:

  • You receive Medicaid Waiver services administered by the Ohio Department of Developmental Disabilities (DODD)
  • You are age 3 or older and ask to have an SSA
  • You live in an Intermediate Care Facility (nursing home or ICF) and ask for help to move from the facility to the community

When you are eligible to receive services from the Wyandot County Board of DD and ask for an SSA, the SSA must be provided to you, and you will not be placed on a waiting list for SSA/Case Management services.  The SSA will assess your needs and provide you with proper resources and services.

Service Monitoring

Service and Support Administration will assist a person and their team in ensuring that services listed in the Individualized Plan are being followed.  SSAs will monitor and follow up with a person and their providers throughout the year to tract progress and make sure that supports are still meeting the person’s needs.  Monitoring happens across all service settings that include but are not limited to a person’s home, their day programming, their provider’s office and in the community.
